Register for Trip

To register your team for a missions activity, please follow these steps:

1. Find Available Date- Contact City on a Hill to ask questions or find an available date. Call 414-931-6670 or email Brooke.

2. Register Group- Download & Complete Registration Form & send deposit to City on a Hill to reserve your date. Choose 1 of the 3 options:

  • Option #1- Send Registration Form to City on a Hill with check for deposit.
  • Option #2- Fax Registration Form to City on a Hill & with Missions Payment Form
         
     6-DAY STM REGISTRATION DEADLINE- STM Deadline is March 31 for summer trip 
                                                                 STM Deadline is December 1st for spring break trips
3. Registration Confirmation- Once the Group Registration Form is received by City on a Hill, City on a Hill will send you a confirmation email with additional information regarding your trip. Your dates is reserved when you receive the confirmation email. Please do not advertise dates with your church until you have received your confirmation notice from City on a Hill.

National AIM Registration (if applicable)- If you're an Assemblies of God church located outside of Wisconsin, please start the AIM Registration process with the National AIM office in Springfield. Download and complete the below forms below:

5. Promote Trip- Visit the Resources page on our website or call Brooke to request promotional DVD.

6. Prepare Your Group- Visit the Missions Trip Handbook section of City on a Hill's website to help prepare your group for trip.

7. Send in 2nd Deposit Payment- By May 1st (February 1st for spring break trips),  confirm your team numbers and pay a second   deposit in the in the amount of $100/person. Make sure to include reference number on the memo of the check. To make deposit by credit card, please use the Missions payment form from Step #2.

8. Send in Participant Release Forms & Applications
    For Poverty Simulation & Urban Plunge - (Collect & give to City on a Hill upon arrival)
          For Adults- Release Form
          For Youth (under 18)- Parent Permission Slip
    For 6-Day Urban Missions Trip - ( Send to City on a Hill by June 1st for Summer Trips or March 1st for Spring Break Trips)
          For Youth, Pastor & Leaders - Participant Application

9. Confirm Group Number- At least 7 days prior to your trip, call City on a Hill to confirm final group numbers, gender breakdown and arrival time.
 
10.  Pay Account Balance- Bring a one check from the church for the balance of your trip with you when you come.  Make sure to include reference number on the memo of the check. If you'd like to send the check to prior to your arrival, please include the Missions Payment Form from Step #2.
