To register your team for a missions activity, please follow these steps:
1. Find Available Date- Contact City on a Hill to ask questions or find an available date. Call 414-931-6670 or email Brooke.
2. Register Group- Complete Registration form & send deposit to City on a Hill to reserve your date. Choose 1 of the 2 options:
3. Registration Confirmation- Once the Group Registration Form is received by City on a Hill, City on a Hill will send you a confirmation email with additional information regarding your trip.
Your dates is reserved when the confirmation email has been sent. Please do not advertise dates with your church until you have received your confirmation notice from City on a Hill.
4. National AIM Registration (if applicable)- If you're an Assemblies of God church located outside of Wisconsin, please start the AIM Registration process with the National AIM office in Springfield. Download the 3 forms below:

5. Promote Trip- Visit the Resources page on our website or call Brooke to request promotional DVD.
6. Complete Participant Release Forms & Applications
For Poverty Simulation & Urban Plunge -
(Collect & give to City on a Hill upon arrival)
For 6-Day Urban Missions Trip -
(
Send to City on a Hill no later than June 1st)
7. Prepare Your Group- Visit the Missions Trip Handbook section of City on a Hill's website to help prepare your group for trip.
8. Confirm Group Number- At least 14 days prior to your trip, call City on a Hill to confirm final group numbers and arrival time.
9. Bring a Check for the balance of your account and give to City on a Hill upon arrival.
