1. Contact Brooke at City on a Hill to find an available date.
2. Once you find an available date, download the MISSIONS GROUP REGISTRATION FORM. Complete and send form to City on a Hill at least 5 weeks prior to desired date with a check for the deposit.
3. Once the agreement form is sent to City on a Hill with the deposit, we will send you a letter, confirming your date. Your date is reserved when the confirmation letter has been sent to you. Please do not advertise dates with your church until this process has been completed.
4. Download & distribute the RELEASE FORM for participants 18 years old & older. Download the PARENT PERMISSION SLIP for participants under the age of 18. ALL PARTICIPANTS ARE REQUIRED TO COMPLETE ONE OF THESE TWO FORMS.
5. To advertise, contact City on a Hill about receiving our logo to help promote your trip.
6. If any changes need to be made regarding your trip, please contact City on a Hill as soon as possible.
7. At least 7 days prior to your trip, call City on a Hill to confirm final group numbers.
8. On the day of the trip, bring all release forms to City on a Hill and give them to your leader.
