The Community Health Worker provides both clinical and administrative support to ensure the efficient operation of health-related services at City on a Hill. The Community Health Worker supports the Medical Providers, Nurse Practitioner, the Health Services Manager, volunteers, and the patients through a variety of tasks related to patient care management, organization, education, and health care navigation. Patient care is provided onsite in the Free Health Clinic, which operates three days per week, and during the monthly Health Outreach Events, which are also held onsite. In addition, the Community Health Worker plays an integral role in the implementation of the Medical Homes for the Homeless Project.
Minimum 3 years of experience in a clinic or hospital setting.
Navigating the healthcare system, insurance background and/or knowledge.
Proficient with navigating the Internet and utilizing Microsoft Office programs including: Word, Excel, Outlook, and PowerPoint.
Strong written, oral, and interpersonal communication skills.
Possesses skills to be highly productive and successful in a fast-paced environment with changing priorities, multi-task duties, and pending deadlines.
Performs expected responsibilities in a self-directed environment.
Demonstrates creative problem-solving abilities by taking initiative, using good judgement, and thinking proactively.
Exudes high energy, a positive attitude, confidence and resilience.
Has strong interpersonal and teamwork skills.

Stands, walks, squats, bends, twists, and reaches above the shoulders.
Able to lift, push, and carry items (ex. bags, boxes etc…) up to 10 or 20 pounds.
